Job Overview:

The Patient Outreach Coordinator plays a key role in supporting patient care, improving health outcomes, and advancing organizational quality initiatives. This position is not only responsible for conducting proactive telephonic outreach to patients and assisting with scheduling, but also helping close care gaps related to HEDIS and other population health measures. The ideal candidate is a patient-centered and detail‑oriented professional who thrives in a high-volume and fast-paced clinical environment.

 Essential Functions:

  • Engage patients through proactive telephonic outreach to facilitate seamless care coordination and enhance the caring quality and patient experience.
  • Streamline patient access to essential care by coordinating primary, specialty, and preventive appointments with clarity and empathy.
  • Collaborate with clinic staff and care management team to address patient inquiries and needs, remove social and clinical barriers, ensure patients receive timely access to necessary care.
  • Contribute to quality improvement activities, including but not limited to closing HEDIS care gaps and optimizing other key clinical quality measures.
  • Participate in population health initiatives, such as preventive care campaigns and wellness programs, to increase patient enrollment and engagement.
  • Follow established clinical workflows and communication protocols to deliver consistent, high-quality patient interactions and ensure service excellence.
  • Assist with other special projects which are related to quality, compliance, and operational improvement.

Qualifications

  • Bilingual in English and Chinese (Mandarin or Cantonese) required
  • High School Diploma or GED
  • Strong communication skills (verbal and written)
  • Excellent interpersonal and relationship-management skills
  • Ability to manage high-volume phone calls while maintaining a high standard of professionalism and patient-centered service
  • Basic computer skills and knowledge of Microsoft Office Suite
  • Proven experience in healthcare, call center, customer service, or patient support setting preferred
  • Hands-on experience with EMR system (MDLand/ECW) preferred
