Department: Rendr 360 is a department of Rendr which provides centralized support to our medical practices: care management, care coordination, Rendr at home, continuous quality improvement, etc. We are an elite clinical division with many patient-centered projects which provide continuity of care throughout the patient’s care journey to help improve clinical outcomes and the overall patient experience. The candidate that takes on this position will focus primarily on Rendr at home, which is part of the Clinical Engagement function. 

Essential Functions:

Clinical Engagement:

  • Provide direct and indirect primary care services to patients with moderate- to high-complexity medical conditions referred inside and outside the network.
  • Perform history and physical evaluation and assessment, assess and diagnose conditions, order and interpret tests, formulate a differential, prescribe treatment plan and manage care via face-to-face clinical programs such as Rendr at home, virtual care, and other face to face clinical platform.
  • Participate in departmental, interdisciplinary, and network committees and meetings as appropriate
  • In collaboration with supervising/collaborating physician, perform direct patient care and manage a range of patient health and wellness issues
  • Other special duties and projects as needed

Clinical Oversight:

  • Participate in all Chronic Care Management activities of patients enrolled in this program in accordance with Centers for Medicare and Medicaid guidelines
  • Develop, assess, and adjust, as necessary, the care plans of patients based upon assessment with specific objectives and interventions to promote desired outcomes
  • Follow-up with patients between primary care and specialty visits, including review of associated lab and radiology reports and specialist recommendations, including pre-authorization of tests and referrals
  • Participate in patient care quality programs such as Transitional Care Management, population health, and continuous quality improvement projects
  • Assess the patient's current health status, resource utilization, past and present treatment plan and services, prognosis, short- and long-term goals, treatment, and provider options
  • Provide patient/family and staff education
  • Facilitate patients' access to network and community-based services

Qualifications:

  • Active NYS APN license required.
  • BLS/ACLS certification required.
  • Previous experience in primary care preferred.
  • Bilingual English and Chinese (Mandarin or Cantonese) required.
