Job Overview:

Rendr360 is a department of Rendr which provides centralized support to our medical practices: care management, care coordination, continuous quality improvement, etc. We are a growing division with many patient-centered projects which provide continuity of care for patients between office visits to help improve clinical outcomes and patient satisfaction. One of these programs is the Transitional Care Management (TCM) Program.

Our TCM Coordinator helps by assisting patients through the transition from extended care facilities (hospital, rehabilitation, nursing facility, etc) to an outpatient setting. They help connect patients to our Rendr360 nurses, schedule appointments, and ensure closure of quality measures.

Essential Functions:

  • Coordinates with internal and external stakeholders in an effort to provide concierge-level service to patients entering our system for the first time, or navigating our system on their journey to wellness.
  • Assists clinicians on various tasks including patient scheduling, patient care notifications, follow-up care, and other tasks involved with aiding in care gap closure.
  • Manages daily admission discharge transfer (ADT) data from our Health Information Exchange (HIE) partners.  
  • Assists a wide variety of stakeholders with various quality, care coordination, and population health projects.

Qualifications

  • Bilingual in English and Chinese (Mandarin or Cantonese) required.
  • A College Degree Preferred
  • Ability to multi-task and use multiple systems, including our EMR to provide medical information and patient support/follow up after the clinical visit.
  • Passionate about high-quality patient care and patient satisfaction.
  • Courteous, empathic, and professional manner.
  • Strong communication, interpersonal, and organizational skills.
  • Prior medical office experience preferred but not required.
  • Some knowledge of medical terminology preferred but not required.
