Job Overview:

Rendr 360 is a department of Rendr that provides centralized support to our medical practices, including care management, care coordination, and continuous quality improvement. As a growing division, we lead patient-centered projects that ensure continuity of care between office visits, improve clinical outcomes, and enhance patient satisfaction. Our initiatives focus on proactive intervention, helping patients manage their conditions effectively to prevent deterioration and improve their health outcomes.

Key Responsibilities:

  • Conduct thorough chart reviews to evaluate patients' current diagnoses, treatment regimens, and medication histories.
  • Evaluate patient’s overall health burden and identify barriers on improving their health outcome.
  • Provide individualized consultation and education on patient’s disease management, ensuring patients understand their conditions and treatment goals.
  • Assist patients in identifying potential signs of deterioration and worsening of their disease states to encourage early intervention.
  • Develop and implement personalized care plans tailored to each patient's needs and health status.
  • Coordinate care efforts with PCPs, specialists, and other members of the patient's care team to ensure seamless and comprehensive treatment.
  • Advocate for patient-centered care by addressing health concerns, improving adherence to treatment regimens, and enhancing overall quality of life.
  • Monitor patient progress, track treatment effectiveness, and make necessary adjustments to care plans in collaboration with healthcare providers.
  • Support patients in navigating healthcare resources, ensuring access to appropriate services and treatments.
  • Perform interdisciplinary processes within Rendr to maximize patient’s health and wellbeing.  

 Qualifications:

  • Bilingual English and Chinese (Mandarin and Cantonese) required.
  • Nursing Degree required.
  • Current NYS Registered Nurse License required.
  • 2+ years clinical nursing experience preferred.
  • Experience in care management, chronic disease management or patient education required.
  • Strong communication, interpersonal, and patient engagement skills.
  • Ability to work within a multidisciplinary team.
  • Ability to multi-task and use multiple systems, including our EMR to provide medical information and patient support/follow up after the clinical visit.
