Job Overview:

Rendr is seeking an Outpatient Nurse Case Manager as a vital advocate for patients, ensuring they receive the care, support, and resources necessary for optimal health and well-being. This newly created role requires a proactive and strategic approach to establish best practices and workflows. The Nurse Case Manager will assess patients' needs at home, coordinate medical care with their providers and specialists, and connect them with essential community resources. This position plays a pivotal role in improving patient outcomes, enhancing access to care, and fostering independence in managing health conditions.

Key Responsibilities:

Patient Advocacy: Serve as the primary advocate for patients, ensuring their healthcare needs are met and supporting informed decision-making.

Home Needs Assessment: Identify patients' at-home care requirements, including medical equipment, home health services, or lifestyle modifications to support health and safety.

Care Coordination: Collaborate with primary care providers, specialists, and the patient’s healthcare team to develop and execute comprehensive care plans.

Transitional Care: Collaborate with primary care providers, specialists, and patients to address necessary care and needs from hospital to in-home setting.

Community Resource Assistance: Assess patient needs and help connect them with local resources, such as financial aid, transportation assistance, housing support, and nutrition programs.

Patient Education: Guide patients on managing their conditions, recognizing early signs of complications, and accessing appropriate care.

Program Development: Establish and refine workflows, protocols, and systems to ensure the success of this new role within the organization.

Quality Improvement: Monitor patient progress and care effectiveness, adjusting strategies to enhance long-term health outcomes.

Collaboration & Outreach: Build relationships with community organizations and healthcare providers to expand resource availability for patients.

Documentation & Reporting: Maintain accurate patient records, document care interventions, and track outcomes to support program growth and effectiveness.

Qualifications:

  • Registered Nurse (RN) license in the State of New York (required).
  • Bachelor’s degree in nursing (BSN) preferred.
  • Minimum of 2 years of clinical nursing experience, preferably in an outpatient, primary care, or case management setting.
  • Minimum 2 years of experience working as a nurse case manager, preferably for a MCO.
  • Strong patient-centered care approach with excellent communication and interpersonal skills.
  • Ability to work independently and manage a caseload efficiently.
  • Proficient in using electronic medical records (EMR) and Microsoft Office applications.
  • Bilingual in English and Chinese (Mandarin and/ or Cantonese).
